Requirements

• Master's degree in Social Work (MSW)
• LMSW or LCSW certification
• Demonstrated knowledge of government and private insurance programs
• Basic computer skills in MS Word, Excel, PowerPoint, and Outlook
• Functional proficiency with DaVita specific applications within 60 days.

Responsibilities

• Support and advocate for patients undergoing dialysis treatment.
• Help patients understand their rights and responsibilities.
• Guide patients in managing the physical, mental, emotional, and financial demands of End Stage Renal Disease.
• Build meaningful and long-term relationships with patients and their families.
• Collaborate with the core care team to improve patient health outcomes.
