THIS JOB IS ELIGIBLE FOR A SIGN ON BONUS! The social worker is a member of an interdisciplinary treatment team responsible for meeting the psychosocial needs of the patient. Responsibilities may include patient/family assessment, counseling, education, case management, advocacy and planning services. The social worker may also facilitate the development of a discharge plan and may coordinate after care continuity. The social worker provides education to staff, patients and families as necessary. Requirements

  • MSW from an accredited graduate school of social work.
  • Current licensure by the State of Connecticut as a Licensed Master Social Worker.
  • Licensed Master Social Worker (LMSW) must achieve (LCSW licensure) within two years of date of hire.
  • Competence in meeting the needs of patients admitted to the hospital for treatment of complex medical issues, chronic disease and/or rehabilitation regardless of age or disability as determined through the interview process, previous work experience, and education.
  • One year clinical experience in a medical setting.

Responsibilities

  • Provide patient/family psychosocial assessment upon admission; ongoing family systems assessment as needed
  • Demonstrates a working knowledge of current theory/practice; integrates such information into practice
  • Completes all documentation according to discipline/unit standards
  • Participates in the development/implementation of an interdisciplinary plan of care
  • Provides supportive counseling to patients/families according to treatment plan
  • Assists in the discharge process in collaboration with the treatment team, patient, and family
  • Develop/maintain an understanding of the culture, traditions, values, beliefs that relate to health care and to decision making
