About The Position

Position Summary: The Social Worker will work in collaboration with physicians and other healthcare professionals to enhance the patient services offered in St. Mary's Hospital's Emergency Department through identifying, evaluating, and development of care plans, based on individual patients' needs for improved patient outcomes. The social worker must have in-depth knowledge of system and community resources and referral agencies. The social worker may also provide assistance to patients, families and others with a wide variety of needs to include, but not limited to, life care planning, financial concerns and/or emotional problems related to chronic and acute illnesses. Provides supportive counseling to patients regarding adjustments to illness as well as other psychosocial needs as indicated.

Requirements

• Holds a current Licensed Master Social Worker (LMSW), Licensed Clinical Social Worker (LCSW) preferred.
• BLS required.
• Must be a graduate of an accredited school of social work, with a minimum of a Master of Social Work
• Must have a minimum of 3 years' experience in social work, with 2 years in a medical outpatient or hospital setting preferred.
• Excellent communication, interpersonal, and critical thinking skills.
• Must be capable of working independently.
