About The Position

The Social Worker / Care Transition Manager at Texas Health Resources is responsible for managing care transitions for patients, particularly in the oncology unit. This full-time position focuses on improving patient outcomes through effective discharge planning and case management, ensuring that patients receive the necessary support and resources as they transition from hospital to home or other care settings.

Requirements

• Master's Degree in Social Work is required.
• LMSW - Licensed Master Social Worker or LCSW - Licensed Clinical Social Worker required upon hire.
• Three (3) years in hospital/medical social work is preferred.
• One (1) year of discharge planning/care management is preferred.
• CPR - Cardiopulmonary Resuscitation is required upon hire.
• ACM - Accredited Case Manager is preferred upon hire.
• CCM - Certified Case Manager is preferred upon hire.

Nice To Haves

• ANCC certification is preferred upon hire.

Responsibilities

• Manage care transitions for patients in the oncology unit.
