JOB DESCRIPTION AND POSITION REQUIREMENTS Scheduled Weekly Hours: 40 8 hr shift - Monday through Friday JOB SUMMARY: The Social Worker intervenes with patients who have complex psychosocial needs, require assistance with eligibility determination for social programs and funding sources and qualify for community assistance from a variety of special funds and agencies. In addition, offer crisis intervention to patients and families with psychosocial needs and coordinates and facilitates the development of a discharge plan of care for high-risk patient populations. This role will receive referrals for individuals from at-risk populations from interdisciplinary team members (including physicians, Case Managers, staff nurses and other members of the care team).

Requirements

  • LMSW or higher licensure required
  • Three or more years hospital social work or protective services experience preferred
  • Working knowledge/experience in utilization management, managed care, and payor issues
  • Experience in psychosocial and therapeutic counseling
  • Ability to demonstrate effective written and verbal communication skills and possess emotional and professional maturity.
  • Basic understanding of Medicare, Medicaid, and Managed Care regulations.
  • Organized and efficient in establishing priorities and performing tasks within specific timeframes.
  • Self-motivated requiring minimal supervision.
  • Views problems as a challenge and investigates alternatives which demand creativity, flexibility, and the ability to work under stress.
  • Responsible for maintaining strict confidentiality of all verbal & written information and documents.
  • Familiarity with and appropriate use of community resources.
