Functions as a member of the PACE Interdisciplinary Team and provides social work services and support to participants, families, and other caregivers. Provides direction to bachelor’s level social workers as assigned by Social Work Manager. The position is anchored in a PACE program at Summit, which is an alternative to nursing home care for individuals 55 and older, allowing them to continue living safely at home. The role includes managing a caseload of only 65 participants in collaboration with a provider and primary care nurse, conducting assessments semi-annually or for significant changes, and assisting with the supervision and training of MSW student interns.
