As the Living Donor Liver Transplant Social Worker, you will provide social work services within the Liver Transplant Service Line. You will conduct psychosocial assessments to learn of each patient's preparedness to donate, facilitate connection with resources to reduce or eliminate barriers to being a living donor and provide supportive counseling. You will also assist with interventions such as referrals to community resources and discharge planning at the time of transplant, as needed. In this role, you will also function as the Independent Living Donor Advocate (ILDA). The purpose of this role is to ensure the protection of current and prospective living donors, provide education to prospective living donors on the transplant process and ensure that there are no elements of coercion to participate in the transplant process. You'll serve as a valuable resource as you connect with patients, their families and the highly skilled interdisciplinary team to ensure excellent outcomes and an outstanding patient experience.
