About The Position

The M Health Fairview Care Coordination team has an immediate opening for a Social Worker-LICSW to support the team at the Otto Bremer Trust Center for Safe & Happy Children located in Minneapolis, MN. Hosted in the Division of Pediatric Emergency Medicine, the Otto Bremer Trust Center for Safe and Healthy Children at M Health Fairview Masonic Children's Hospital is dedicated to providing medical care for victims and potential victims of child abuse and neglect. We provide comprehensive consultation, education and support for medical providers, community agencies and caregivers. We see children in hospital settings including inpatient units, emergency departments and specialty clinics. The Center for Safe and Healthy Children includes a Medical Director subspecialty-certified in Child Abuse Pediatrics and a program coordinator licensed in clinical social work as well as additional dedicated staff consisting of a child abuse pediatrician, pediatric nurse practitioner, research coordinator, clinical social worker, fellowship coordinator, executive assistant, administrative supervisor, and a certified therapy dog. This role covers clinic days as well as inpatient and emergency room consults to SAFE clinic. Completes mental health assessments, crisis evaluation and counseling and psychosocial assessment of patients, for post hospital discharge needs. Assists patients with short-term counseling and planning following medical crisis. Ensures appropriate decision makers are informed of and involved in treatment planning.

Requirements

  • Master of Social Work from an accredited school
  • 3 years hospital social work experience. This experience may be substituted with direct clinical experience
  • Knowledge of DSM IV categories and criteria
  • Licensed Independent Clinical Social Worker (LICSW)

Responsibilities

  • Maintains knowledge of, and complies with, all relevant laws, regulations, policies, procedures, and standards.
  • Actively participates in creating and implementing improvements.
  • Evaluates psychosocial information (including social, psychosocial, emotional, financial, and other related factors) to determine an appropriate action/discharge plan.
  • Assists physician with diagnoses related to mental health and chemical dependency.
  • Documents patient information in EHR and/or other electronic system in accordance with program standards, department policies/procedures.
  • Assists patient/family in utilizing appropriate community resources.
  • Performs supportive counseling to assist patient/families in adjusting to disability and illness and for realistic planning for post hospital care.
  • Completes coding of assessments and evaluations for billing.
