About The Position

The Licensed Independent Clinical Social Worker (LICSW) is responsible for the social, emotional, and financial wellbeing of patients on the Behavioral Health Units. The LICSW will participate in interdisciplinary treatment team planning; is responsible for discharge planning, running treatment groups as assigned, and performing appropriate psychosocial assessments and interventions. The LICSW will assist with referrals to the unit and make referrals to appropriate community agencies as indicated. Requirements

  • Master’s Degree in Social work or psychiatric case management years
  • Minimum of 3k and a equivalent combination of education and experience
  • Current LICSW in the state of Massachusetts
  • Strong knowledge of DSM diagnoses including dementia diagnoses
  • Knowledge of medical terminology
  • Excellent organizational skills and ability to multi-task in a fast-paced environment
  • Case management skills
  • Knowledge of DMH regulatory requirements

Nice To Haves

  • 4-5 years of experience working with patients and families who are in crisis.

Responsibilities

  • Supervises and trains graduate level social work students and other students when indicated and approved by the Director of Social Work.
  • Develop an individualized plan of care with interdisciplinary treatment team
  • Coordinate discharge/follow-up activities
  • Discuss with patients/families the financial implications of placement decisions and provide
  • Provide individualized psychotherapy, supportive counseling, and group therapy as indicated
  • Make referrals to community agencies as appropriate
  • Identify potential legal/risk management issues and review with Program Director
  • Document, in timely fashion, initial assessment, proposed plan, treatment updates, and final discharge in electronic medical record
  • Communicates with staff, nursing facilities, agencies, and outpatient providers as it relates to patient treatment plan, discharge or behavioral plan.
  • Educate patients/families, staff and providers regarding managed care and Medicare processes/regulations
  • Maintain current knowledge of DMH and applicable mental health legal status/advance directive regulatory requirements
  • Provide disease state teaching to patients/families
  • Provide education/consultation to other healthcare team members as needed
  • Collects statistical data as requested by the Director of Social Work
  • Regularly attends/participates in treatment team rounds
  • Performs other duties as assigned.
