About The Position

We are seeking a Licensed Clinical Social Worker (LCSW) to join our behavioral health team on a contract basis. The ideal candidate will provide comprehensive clinical assessments, therapeutic services, and discharge planning in collaboration with a multidisciplinary team, ensuring patients receive the highest quality of care both during and after hospitalization.

Requirements

  • Licensure: Active Massachusetts Licensed Clinical Social Worker (LCSW) – Required
  • Certification: Basic Life Support (BLS) – Required
  • Strong knowledge of mental health regulations and care coordination processes.
  • Excellent communication, documentation, and interpersonal skills.
  • Ability to work collaboratively in a multidisciplinary team environment.

Responsibilities

  • Conduct initial clinical assessments for individuals with acute care needs in compliance with Massachusetts Department of Mental Health (DMH) requirements.
  • Develop and present Initial Treatment Plans based on assessment findings to the treatment team.
  • Lead treatment and discharge planning in collaboration with psychiatrists and other healthcare providers.
  • Provide group therapy, individual counseling, family counseling, and other therapeutic interventions.
  • Support patients in ongoing treatment planning for optimal recovery and well-being.
  • Assess medical and psychosocial needs, coordinating home care and community services.
  • Connect patients with community programs, resources, and post-discharge support.
  • Assist patients and families in obtaining necessary medical equipment or placement in appropriate facilities.
  • Collaborate with community-based agencies and service providers.
  • Deliver crisis prevention and intervention services, particularly during care transitions.
  • Provide coverage for Inpatient and Partial Hospital Program clinicians as needed.
  • Communicate with managed care companies to obtain care authorizations.
  • Maintain up-to-date knowledge of CMS, DPH, DMH, and other regulatory standards.
  • Participate in community outreach and educational initiatives.
  • Consistently demonstrate service excellence to patients, families, and colleagues.
  • Perform additional related duties as assigned.
