About The Position

This position involves traveling to see hospice patients in the Portland Metro service areas. The Licensed Clinical Social Worker (LCSW) independently provides comprehensive biopsychosocial/spiritual assessment, diagnosis, treatment, interventions, and advocacy for patients and their support systems. The LCSW incorporates concepts and theories related to life span development, neurobiology, and behavioral change. Collaboration with the patient and support system is essential to develop and implement evidence-informed person-centered care plans across the care continuum, in conjunction with the Inter/Multi-Disciplinary Team (IDT). The LCSW is expected to perform duties respecting patient self-determination, reducing health disparities/barriers to care, and adhering to the NASW code of ethics, Standards of Practice (SOP), and the mission of Providence. Requirements

  • Master's Degree in Social Work from a CSWE Accredited School of Social Work, OR from a school in candidacy status by the council of Social Work Education (CSWE)
  • 2 years relevant Social Work experience (may include CSWA experience)
  • Oregon Clinical Social Worker License/Clinical Social Work Associate License upon hire
  • Washington Clinical Social Worker License or affiliated counselor credential within 2 months of hire
  • Driving is a necessary part of this role
  • Comply with all state laws and requirements for driving
  • Provide proof of driver license and auto insurance upon request

Nice To Haves

  • 1 year Care Management experience in acute care settings
