As our Social Worker, you will be a compassionate advocate, providing vital support and guidance to individuals and families facing complex challenges. Every day, you will conduct psychosocial assessments, develop care plans, provide counseling and crisis intervention, and connect clients to resources. You'll advocate for clients and collaborate with multidisciplinary teams to ensure holistic support and promote well-being. To be successful in this role, you will possess exceptional interpersonal skills, empathy, strong knowledge of community resources, and proven crisis management abilities, fostering positive change and client empowerment. Responds to referral within 24 hours. Common referrals include: placement issues, patient/customer dissatisfaction, OB needs assessment, death & dying, chemical dependency, bioethical issues, newly diagnosed chronic diseases, crisis intervention, and case management assessment/discharge screening Extracts pertinent information from the chart and documents information in a concise and legible manner. Documentation includes: 1. An assessment of the patients physical, functional, psychological, social, environmental & financial status. 2. Assessment of the patients informal & formal support systems. 3. Initial and ongoing discharge planning assessment Responds to ER referrals in a timely & complete manner Assists patient/family in completion of CMSP or MediCal applications Exhibits a working knowledge of DRG concepts & managed care contracts, CMSP, MediCal & other reimbursement programs Communicates ongoing discharge plan and pertinent psychosocial issues to all caregivers over the continuum.
