One California Job Summary and Responsibilities As our Social Worker, you will be a compassionate advocate, providing vital support and guidance to individuals and families facing complex challenges. Every day, you will conduct psychosocial assessments, develop care plans, provide counseling and crisis intervention, and connect clients to resources. You'll advocate for clients and collaborate with multidisciplinary teams to ensure holistic support and promote well-being. To be successful in this role, you will possess exceptional interpersonal skills, empathy, strong knowledge of community resources, and proven crisis management abilities, fostering positive change and client empowerment. Primary discharge planning contact person within the adult patient social work team, providing coordinated continuity-of-care communication between social workers, physicians, agencies, third-party payers, families and patients. May schedule and facilitate team conferences/staffings, prepare patient and families for participation in conferences and/or other team coordination activities, as assigned. Interviews patients and families for psychosocial assessment and treatment planning needs. Develops and implements age and disability appropriate psychosocial treatment plans related to personal and family adjustment and continuity of care needs, through individual, family and group therapy and educational/support sessions. Participates in conferences/staffings and consultations with team to provide information and make recommendations for patient's treatment, discharge planning and milieu management. Provides information, referrals and liaison activities with third party payor and community resources necessary for patient's post hospital needs.
