The Outpatient LCSW works collaboratively with the interdisciplinary team, to assess the needs of patients and their family/caregivers when appropriate, and arranges, coordinates, and advocates for services to meet the members physical and psychosocial needs. The social worker provides education to staff, patients and families as necessary At HFSC, we are a family. Requirements

  • M.S.W. from an accredited graduate school of social work.
  • Current licensure by the State of Connecticut as a Clinical Social Worker (LCSW).
  • Competence in meeting the needs of patients who require treatment for chronic disease, complex medical issues and/or rehab therapies.

Nice To Haves

  • One-year clinical experience in a medical setting.

Responsibilities

  • Establishes and maintains effective working relationships with community providers, social service agencies and members of the interdisciplinary team.
  • Maintains flexibility and adjusts approach to the needs of the team.
  • Researches and arranges community resources for patients/caregivers.
  • Provides supportive counseling to patients/families to address psychosocial issues that may affect the plan of care established by the team.
  • Runs patient group meetings specific to disease or identified psychosocial needs to support the outpatient population.
  • Completes documentation in the medical record in a timely manner and in keeping with, internal policy and professional standards.
  • Develop/maintain an understanding of the culture, traditions, values, beliefs that relate to health care and to decision making.
