About The Position

As a Social Worker, LCSW you will perform activities including admitting, assessing, and carrying out a schedule of individual or group family therapy as part of the agency's Intermediate Care Facility. Essential Duties and Responsibilities include the following. Other duties may be assigned. Performs various psychosocial intakes calling for interviewing, completing appropriate history and intake forms and assessing other aspects of the potential individual’s circumstances and background. Prepares intake charts for treatment planning for other disciplines—PT/OT/Speech/Social Work/Psychology. Provides assessments, investigating the consumer's history, nature of the problems, and disabilities and other pertinent circumstances. Pursues specific path of therapy based on assessments of plans for and arranges referrals to other professionals as indications suggest. Assessments are carried out in a variety of settings, including the home. Prepares and maintains a broad variety of documentation and records, including writing of Comprehensive Functional Assessments, treatment plans, behavior plans, case reports, and all related details or recommendations, therapy provided, etc. Summarizes significant events, changes in health or family situations and other relevant information. Prepares clinical notes for each individual and other required reports and records for outside organizations and for internal use and record keeping by the agency. Participates as member of the individual’s treatment team. Reports psychosocial information, reviews and recommends goals, analyses problems presented by other professionals, etc. Participates in analyzing each consumer's needs and in the modification and establishment of new goals. Maintains tracking of Individuals supported, provide necessary information to Fisher team to facilitate consumer's admission to and maintenance in Residence. Oversees the performance and activities of the individual in the ICF, providing guidance and direction as part of the training and educational program. Completes appropriate records and forms documenting individuals’ activities as required. Provides individual and group therapy for the residents of the ICF; as well as residents of the IRAs if needed. Documents clinical intervention, writes treatment plans, and psychiatric consultation sheets. Prepares and submits billing forms. May provide family counseling/intervention. Conducts therapeutic group services for residents of the ICF Participate as member of the Admissions Committee, screening individuals for residential vacancies and for development of new residences with the Admissions team, as needed. Based on business needs, participates as a member of the Human Rights Committee. Informed Consent Committee, and Incident Review Committee. Facilitate Family/Advocate meeting as required Facilitate family groups as requested Facilitate staff support groups as needed in times of loss Perform other routine administrative functions related to the Fisher ICF. May perform a variety of clerical and routine administrative functions Maintain safe, orderly work area and work practices. Performs other duties as required. Article 16 Responsibilities Provide counseling/psychotherapy services to individuals and their families Manage scheduling for Article 16 clinical cases. Complete required Article 16 clinical documentation within established timeframes. Assist with preparing needed clinical information to apply for service authorization as needed. Assist Article 16 Program Director with screening Social Work cases for appropriateness for admission to Article 16 clinic #INDBH

Requirements

  • LCSW strongly preferred or master’s degree in social work and one year experience with OPWDD population.
  • NYS certification as Social Worker.
