Description Provides social work and behavioral health services for medical home clinic patients collaboratively with care team, in accordance with policy and procedures of St. Patrick Hospital and Health Sciences Center and Providence Medical Group, and in collaboration with the patient and patient's family and/or caregiver. Requirements
- Master's Degree in Social Work
- Montana Clinical Social Worker License upon hire
- 3 years of experience with adult populations and familiarity with broad range of psychiatric disorders.
Nice To Haves
- OSW-C certification (for the Cancer Center LCSW) within 5 years of hire.
- Prior oncology experience preferred for the LCSW in the Cancer Center
