The Palliative Care Social Worker is responsible for coordinating an interdisciplinary approach to deliver palliative healthcare services to patients. This role involves planning, organizing, and implementing Social Work services that address the physical, psycho-social, spiritual, cultural, developmental, and age-appropriate needs of patients and their families. The Social Worker ensures that appropriate decision makers are informed of and involved in treatment planning. Additionally, the Palliative Care Social Worker applies pertinent state and federal regulations regarding discharge planning and reporting requirements. They also identify, monitor, and report opportunities for quality and performance improvement to the appropriate departments and take an active role in performance improvement activities related to their area of assignment.
