About The Position

The Social Worker LCSW for Outpatient Transplant at Tampa General Hospital is responsible for managing complex psychosocial and economic co-morbidities to create optimal outcomes for patients and their families. This role involves providing social work services, coordinating transitions of care, and ensuring effective communication among healthcare team members to facilitate patient care and resource mobilization.

Requirements

  • Master of Social Work (MSW) degree from a C.S.W.E.-accredited program.
  • Licensed Clinical Social Worker (LCSW) certification is required.
  • At least 2 years of experience in a healthcare facility or program.
  • Experience in transplant services, particularly kidney transplant and hemodialysis, is preferred.

Nice To Haves

  • Experience with crisis intervention and addiction assessments.
  • Familiarity with community resources for psychiatric referrals.

Responsibilities

  • Manage complex psychosocial and economic co-morbidities for patients and families.
  • Provide social work services to optimize patient treatment and coping mechanisms.
  • Coordinate transitions of care for patients with high psychosocial needs.
  • Develop and maintain knowledge of local, state, and federal services for patient education and resources.
  • Ensure timely and accurate data entry in multiple information systems.
  • Collaborate with Care Manager and interdisciplinary team members on the multidisciplinary plan of care.
  • Attend Multidisciplinary Rounds or Huddles to report on psychosocial status.
  • Monitor progress toward social work plan goals and revise as needed.
  • Assess potential obstacles to community reintegration and escalate issues as necessary.
  • Communicate and collaborate effectively with the healthcare team to facilitate patient care.
  • Counsel patients and families on emotional, social, and financial consequences of illness.
  • Provide brief crisis counseling and therapeutic interventions for frequent healthcare utilizers.
  • Conduct post-discharge follow-up calls to prevent readmission for high-needs patients.
  • Complete psychosocial assessments and develop short-term treatment plans.
  • Identify and conduct family meetings to achieve important outcomes.
  • Lead efforts in obtaining charity and financial resources, legal guardianship, and psychiatric referrals.
  • Provide intervention for end-of-life issues, guardianship, and domestic violence cases.
  • Participate in Care Coordination Rounds and clinical performance improvement activities.
