Join Mercy Care Atlanta as a Social Worker (Senior Behavioral Health Specialist), where you'll provide compassionate, comprehensive behavioral health services. Under the guidance of the Coordinator of Behavioral Health Services, you will enhance patient care through psychosocial assessments, therapy, and consultation. Your role will be pivotal in improving psychosocial functioning and providing integrated care within an interdisciplinary team. What You Will Do: Perform psychosocial assessments and create treatment plans. Provide counseling, therapy, and education to patients. Execute involuntary commitment orders and manage crisis situations. Offer face-to-face linkage and ongoing case management services. Collaborate with primary care teams to develop care management processes. Participate in interdisciplinary meetings and community health events. Educate patients, families, and staff on behavioral health care and prevention. Provide clinical guidance and support to the behavioral health services team. Carries out administrative duties related to the overall Project LAUNCH requirement and reporting measures, behavioral health strategic plan, as well as functions as a direct behavioral health service provider on team including assessing, therapy, and conducting groups Works directly with community service organizations to develop and collaboratively utilize community resources and lead the (YCWC) Young Child Wellness Council Manages assigned budgets and completes accurate and timely reports on program activities.
