Requirements

  • Requires a Master of Social Work (MSW) or Master of Science in Social Administration (MSSA) degree from an accredited social work program.
  • Advanced knowledge of mental illness and personality development.
  • Knowledge of the types of treatment and skills in conducting individual, group, and family therapy.
  • Knowledge of adjustment to medical diagnoses and chronic disease.
  • Strong communication skills to interact with patients, family members, consultants, residents, hospital personnel, community agencies, and other resources.
  • Psychosocial diagnostic skills and the ability to communicate effectively in both verbal and written form.
  • Licensure (LCSW) to practice independently at the clinical level as required by applicable state requirements.

Responsibilities

  • Collaborates with multidisciplinary teams.
  • Provides counseling and/or psychotherapy.
  • Coordinates patient discharge planning.
  • Complies with required standards for medical/legal responsibilities.
  • Serves as a resource to other allied and administrative staff within Outpatient Social Work and the Department of Care Management.
  • Provides psychosocial assessments, emotional support, and counseling to help patients cope with the challenges of waiting for, undergoing, and recovering from heart transplantation.
  • Evaluates patients' social support systems, financial resources, and mental health status.
  • Ensures readiness for transplant and long-term adherence to post-operative care.
  • Assists with resource coordination, including housing, transportation, insurance, and access to community services.
  • Advocates for the patient's overall well-being and quality of life.
  • Facilitates a support group, education programs, community events, and more.
  • Accesses a strong team of clinical social workers, committee and special project opportunities.
  • Serves as preceptor in the internship and fellowship program.
  • Works towards obtaining academic rank.
