About The Position

The position involves planning, directing, defining, and coordinating individualized and group therapeutic support and services to patients with behavioral problems, psychiatric disorders, post-traumatic stress disorder, substance use, and physical and emotional abuse. The role requires conducting psychosocial assessments, developing effective treatment plans, and ensuring patient safety at all times.

Requirements

  • Master's degree in Social Work from an accredited college or university.
  • Three or more years of related experience working with a comparable patient population.
  • Supervisory experience preferred if responsible for supervising staff.
  • State licensure as required by the state in which the facility operates (LMSW, LCSW, LSW).
  • CPR and de-escalation/restraint certification required (training available upon hire).
  • First aid certification may be required based on state or facility.

Responsibilities

  • Conduct psychosocial assessment of patient and develop an effective treatment plan.
  • Obtain information on the patient's needs, skills, interests, and limitations through patient observation and interviews.
  • Provide crisis intervention and assist families in understanding the implications of life situations.
  • Participate in multidisciplinary treatment team meetings for assessment and treatment planning.
  • Identify, assess, and coordinate patient cases related to illness, hospitalization, finances, education, and community services.
  • Facilitate education, process groups, support groups, and referrals as requested.
  • Conduct intakes and assist in coordinating placements within facility programs.
  • Develop and execute clinically relevant activities in coordination with activity professionals.
  • Promote recovery, symptom reduction, and social functioning.
  • Communicate with family members to support the patient.
  • Manage a caseload and ensure accurate patient chart documentation.
  • Supervise social work interns and provide guidance to social services staff.
  • Lead the program/unit and coordinate activities of clinical or direct patient care staff.
  • Demonstrate a positive, empathetic, and professional attitude towards customers.
  • Acknowledge and resolve complaints when patient needs are not met.
  • Recognize that patient safety is a top priority.
  • Perform other functions and tasks as assigned.
