About The Position

As an Inpatient Social Worker, you will support patients and their families during some of their most challenging moments—providing compassionate guidance, coordinating services, and ensuring a smooth transition through the continuum of care. Working closely with a multidisciplinary team, you’ll help address psychosocial needs, connect patients with essential community resources, and advocate for the support systems that promote healing and long‑term well‑being.

Requirements

  • Master’s degree in Social Work from a CSWE‑accredited program (required).

Nice To Haves

  • Clinical licensure or provisional licensure (desirable).
  • Accredited Case Manager (ACM‑SW) certification preferred.
  • Prior social work experience in a healthcare setting strongly preferred.

Responsibilities

  • Identify patients who would benefit from Case Management interventions through initial screening of discharge needs.
  • Conduct comprehensive psychosocial assessments to understand emotional, social, and environmental factors affecting health and treatment.
  • Gather information through patient/family interviews and objective evaluation of needs.
  • Formulate and implement individualized psychosocial treatment plans using appropriate social work modalities.
  • Provide crisis intervention, individual and family therapy, and grief/bereavement counseling as needed.
  • Initiate internal and external referrals based on patient‑specific needs.
  • Maintain current knowledge of relevant federal, state, and local regulations impacting patient care.
  • Advocate for patients with unmet needs to ensure access to appropriate resources and services.
  • Collaborate with the multidisciplinary team to develop effective discharge plans and support timely progression of care.
  • Stay informed about payer and reimbursement practices influencing care planning and resource utilization.
  • Coordinate patient and family care conferences when appropriate.
  • Identify and work to remove barriers to effective patient care management.
  • Document all assessments, plans, and interventions in the Electronic Medical Record according to departmental standards.
  • Guide patients and families in exploring post‑discharge care options.
  • Demonstrate competency in providing age‑ and developmentally appropriate care.
  • Foster positive working relationships and contribute to a collaborative, conflict‑aware environment.
  • Adapt effectively to fast-paced settings with frequent interruptions and evolving healthcare environments.
