The Cardiac Medicine Social Worker works with high acuity patients and their families on a cardiac medicine floor. This role offers the opportunity to utilize brief counseling skills to support adjustment to illness and patient engagement in their treatment. The social worker will collaborate closely with physician, nursing, rehab, and case management colleagues, as well as community-based agencies, to facilitate transitions to appropriate levels of care and optimize treatment outcomes. Clinical supervision and professional development are highly valued and provided, including continuing education and Social Work Grand Rounds on site.
