Are you a compassionate MSW with a passion for providing exceptional end-of-life care? In this role, you will play a crucial part in supporting patients and their families through the challenging stages of Chronic Kidney Disease (CKD) and End Stage Kidney Disease (ESKD), ensuring they receive the highest quality of care and emotional support. This is a hybrid role where you will provide virtual patient support and lead a team of care coordinators from your home office, with occasional travel to see patients. You will work closely with cross-functional teams to deliver high-quality patient care and have opportunities for career growth and development within our organization.
