About The Position

The Division of Developmental Disabilities is seeking to hire a Social Worker III for the Ludeman Developmental Center located in Park Forest, Illinois to serve as professional Social Worker providing services to a caseload. Oversees admission, discharge and transitional services. Performs complex social work services for individuals on a facility-wide basis. Serves as a lead worker to other social work staff. Serves as a member of Interdisciplinary (ID) Teams. Co-chairs the Professional Staff Team. Coordinates placement efforts and acts as a facility resource for Affirmative Action, Equal Employment Opportunity, Americans with Disabilities Act, and Employee Assistance Program.

Requirements

  • Requires a master’s degree in social work from a recognized college or university supplemented by two (2) years of professional social work experience in the evaluation and treatment/habilitation of mentally ill, developmentally disabled or other disabled persons.
  • Requires the ability to physically restrain individuals as medically necessary to prevent injury to the patient or others.
  • Requires the ability to work after business hours, weekends and holidays.
  • Requires the ability to serve in a leadership capacity to less experienced professional care staff.
  • Requires the ability to prepare complex written and oral reports.
  • Requires ability to pass the IDHS background check.
  • Requires ability to pass a drug screen for drugs prohibited from recreational use under Illinois Law.

Responsibilities

  • Serves as professional Social Worker providing services to a caseload of individuals that are being considered for community placement.
  • Serves as the primary team liaison with other disciplines, guardians, residential unit staff, family and community agencies in individual admission and community placement processes.
  • Serves as designated lead worker.
  • Conducts interviews with individuals, family members and other persons to obtain a comprehensive individual and family evaluation and assessment for each individual, this includes the preparation of a social history and periodic updates.
  • Coordinates the decision-making for individual habilitation and treatment issues.
  • Documents the Center’s efforts in transition planning activities by collecting, organizing, and maintaining accurate and up-to-date data.
  • Performs other duties as required or assigned which are reasonably within the scope of the duties enumerated above.
