About The Position

Step into a role where your clinical insight, empathy, and problem‑solving skills come together to support patients during some of their most vulnerable moments. As a Medical Social Worker, you will provide essential psychosocial care, guide patients and families through complex decisions, and collaborate across the interdisciplinary team to ensure safe, effective, and compassionate care throughout the hospital stay and beyond. You’ll serve as both advocate and educator—helping patients cope, navigate resources, and transition confidently to the next phase of care.

Requirements

  • Master’s degree in Social Work (MSW) required.
  • Adherence to the National Association of Social Workers (NASW) Code of Ethics required.
  • Must meet physical requirements, including ability to work in fast‑paced environments, walk extensively, and perform documentation for prolonged periods.

Nice To Haves

  • LCSW preferred; applicable state certification preferred.
  • At least 1 year of experience in a hospital or health‑related setting preferred.
  • Familiarity with or proficiency in data management tools is preferred.

Responsibilities

  • Provide psychosocial assessments, counseling, crisis intervention, and information/referral services to patients and families on a consultation basis.
  • Complete comprehensive psychosocial assessments to support post‑hospital care planning for designated patients and their support systems.
  • Assist patients in coping with stress related to hospitalization, disability, chronic illness, or terminal diagnoses.
  • Collaborate with the Clinical Care Management team to evaluate and recommend the appropriate level of care or care environment across the continuum.
  • Serve as a resource to hospital staff and physicians regarding emotional, social, and psychosocial components of illness and their impact on support systems.
  • Provide age‑appropriate education on community resources and post‑hospital options; make referrals to community agencies as needed.
  • Report suspected child or adult abuse, neglect, or exploitation, and serve as liaison with the Department of Social Services during evaluation/investigation.
  • Respond to requests for after‑hours consultative services as needed.
  • Promote optimal clinical and resource outcomes by supporting length‑of‑stay management, reimbursement goals, throughput, and efficient use of resources.
  • Act as a central information and education resource for the interdisciplinary team; identify system variances and contribute to organization‑wide problem‑solving and quality improvement.
  • Support discharge planning and transitions, ensuring continuity of care and reducing avoidable delays.
