Lower Bucks Hospital is a community hospital, serving the Lower Bucks County region with medical, surgical, and emergency care. Located in historic Bristol, Pennsylvania, the Hospital is well-known for its knowledgeable medical staff, including cardiology, orthopedics, radiology, wound care, and general surgery. The hospital has more than 200 physicians on staff, as well as another 700 employees. The Social Services II role involves providing emotional support, crisis intervention, and counseling for Pediatrics, Adolescents, Adults, Older Adults, and their significant others to help them cope with social and emotional problems. This includes information and referral services, and referrals for financial assistance and supportive services to patients and families with problems that are related to hospitalization, illness, disability, or trauma. The position also entails case management, milieu management, completing psychosocial assessments, assisting in the development and re-evaluation of Treatment Plans, working as a member of a multidisciplinary team, and coordinating discharge planning. Necessary documentation and clinical record maintenance for each client during their length of stay are also required.

Requirements

  • Master's Degree in Social Work related field OR Master's Degree in Marriage and Family Therapy or related field
  • In states without required licensure, then current registration with State Board is required within 3 months of hire
  • In states with required licensure, license required at time of hire and must be maintained
  • Behavioral Violence Prevention Training within 3 months of hire and maintain current
  • Minimum one (1) year hospital or Outpatient experience with emphasis in mental health care
  • Experience in working with a diverse population with a wide range of problems (i.e., death and dying, maternal and child care, substance abuse, disability, chronic illness, etc.)

Nice To Haves

  • Current BCLS (AHA) certificate upon hire and maintain current preferred

Responsibilities
