About The Position

Baptist Health is currently seeking a Master's Level Social Worker (Social Worker II) to join our team on a PRN, day shift basis. This role will provide inpatient social work support across all Baptist Health campuses, working as part of an interdisciplinary care team. As a Social Worker II, you will play a vital role in enhancing patient well-being by addressing psychosocial needs, reducing barriers to care, and connecting patients with appropriate community resources. Your work will help alleviate the burdens associated with medical conditions and support holistic patient care.

Requirements

• Master's degree in Social Work (MSW) is required
• 3-5 Years Discharge Planning Experience
• 3-5 Years Case Management Experience

Nice To Haves

• Licensed Clinical Social Worker Preferred
• Doctorate

Responsibilities

• Assess psychosocial needs and provide supportive services to enhance patient functioning and emotional well-being
• Deliver care coordination, psychoeducation, and referrals to community resources
