About The Position

Provide social work services, medical case management, and care coordination to patients and their families. Identify needed community services and refer patients in need to the appropriate services. Collaborate with other members of the multi-disciplinary team to affect positive patient and family outcomes.

Responsibilities

  • Identifies patients who would benefit from Case Management interventions based on an initial screening assessment.
  • Assesses all relevant data and obtains information by interviewing patient/family and performing objective evaluation of patient needs.
  • Completes comprehensive and individualized care plans and acuity scores ensuring continuous client monitoring for progress of treatment goals.
  • Re-evaluation of the care plan and acuity score must be in accordance with Ryan White policy.
  • Initiates appropriate internal and external referrals specific to individual patient needs. Healthcare Marketplace.
  • Facilitates access to medication by collaborating with the Patient Navigator team and completing applications for HMAP, Co-Payment Assistance, Patient Assistance and 340B programs ensuring requirements are met for enrollment and choice is provided to the patient.
  • Utilize treatment and prevention strategies by collaborating with the Patient Navigators, Triage Nurses, and Disease Intervention Specialist Bridge Counselors to ensure those lost to care are provided support to make informed decisions about continued care.
  • Attends and actively participates in weekly multi-disciplinary team meetings, clinical conferences, staff meetings, assigned committees, and task forces
  • Maintains current knowledge of available federal, state, and local regulations.
  • Advocates for unmet needs on behalf of patients.
  • Participates in the development of a discharge plan in collaboration with the multidisciplinary team to drive patient progression through the continuum of care.
  • Complete all HIV certification training programs and medical case management related training as required by the NC HIV Care Program.
  • Maintains current knowledge and awareness of payer and reimbursement practices impacting the plan of care.
  • Coordinates patient and family care conferences as needed.
  • Identifies barriers to efficient and effective management of patient care and seeks strategies to eliminate the barriers.
  • Documents in in the Electronic Medical Record in accordance with departmental reporting standards.
