Responsibilities

• Collaborate with medical providers and community partners to address client needs and monitor case progress.
• Conduct presentations and attend community events to educate the public about case management services.
• Provide technical assistance and training to service providers and contractors.
• Participate in community organization functions and collaborate with other entities on health and social work issues.
• Maintain documentation of work activities, including reports and case summaries.
• Participate in disaster response and recovery efforts as needed.
