Requirements

  • Requires a master's degree in social work from a recognized college or university supplemented by one (1) year of professional social work experience in the evaluation and treatment/habilitation of mentally ill, developmentally disabled, or other disabled persons.
  • May require the School Social Worker Endorsement from the Illinois State Board of Education.
  • When hiring from an open competitive list, candidates who are licensed clinical social workers may be preferred.
  • This class is included as an Upward Mobility Program credential title.
  • Requires the ability to pass a background check.
  • Requires the ability to pass a drug screen and pre-employment physical.
  • Requires the ability to meet all agency vaccine/health-related policies and guidance.
  • Requires ability acquire and maintain Cardiopulmonary Resuscitation (CPR) certificate.
  • Requires the ability to utilize office equipment, including personal computers.
  • Requires ability to attend seminars, conferences, and trainings to stay current on methods, tools, ideologies, or other industry related topics relevant to the job duties.
  • Requires ability to travel in the performance of duties, with overnight stays as appropriate.
  • Overtime is a condition or employment, and you may be required to work overtime including scheduled, unscheduled, or last-minute overtime.

  • Prefers at least one (1) year of experience assisting with plans for behavioral changes.

Responsibilities

  • Provides professional social work services to residents through the casework process with personal problems related to retirement, aging, and separation from their home communities.
  • Coordinates the treatment and goals for assigned caseload, including those of an advanced complexity level.
  • Completes assessments or Initial Social Service Evaluations to include Mental Status Exam, Depression Scale and Suicide Risk for all new resident admissions to the facility.
  • In compliance with Medicare and HIPAA regulations, provides individual and/or group psychotherapy services, to enable each resident to live in the least restrictive environment possible.
  • Assists residents with adjustment to changes in lifestyle, health and aging through individual and/or group therapeutic processes.
  • Performs and/or assists with crisis intervention/conflict resolution with residents, families and/or staff and assists residents with discharge plans.
  • Prepares social history and other consents and releases on all newly admitted residents for use by the interdisciplinary team to provide information regarding personal/family history of mental health issues, spirituality, and strengths.
  • Assists nursing supervisors with resident interviews concerning complaints, grievances, allegations of abuse, neglect, or misappropriations.
  • Serves as a liaison between families and caregivers.
  • Serves as a resource person to Social Service Program Planners and provides training/in-services as needed.
