About The Position

The Social Worker II position at the Colorado Mental Health Hospital in Fort Logan is responsible for providing therapeutic services to individuals with severe and persistent mental illnesses. Requirements

  • Current, valid Colorado or State participating in the Social Work Licensure Compact licensure as a Licensed Social Worker (LSW), Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), or Licensed Marriage and Family Therapist (LMFT).
  • Ability to complete biopsychosocial assessments within designated timeframes and maintain confidentiality.
  • Experience in providing psychoeducation to support systems and community stakeholders.

Nice To Haves

  • Licensure as LSW or LCSW preferred.
  • Knowledge of DSM 5-TR.
  • Prior adult inpatient experience.
  • Desire to work with adults diagnosed with severe and persistent mental illness.

Responsibilities

  • Provide individual therapy, group therapy, and family therapy as needed based on treatment plans.
  • Develop group materials tailored to patient-specific needs and lead group therapy sessions.
  • Complete comprehensive biopsychosocial assessments and reassessments for all patients admitted for care.
  • Coordinate treatment planning and document progress in the electronic health record.
  • Manage a treatment caseload and ensure timely coordination of resources for discharge planning.
  • Collaborate with treatment teams, families, and external stakeholders to support continuity of care.
  • Attend interagency staff meetings and provide education to stakeholders about the Social Work Department.
  • Contribute to hospital-wide events, in-service training, and program development.
