Assists in a multi-disciplinary approach to patient care. May identify discharge needs and arrange for comprehensive services for patients following discharge from the hospital. Performs related responsibilities as required. ADDITIONAL JOB DETAILS: This position is scheduled at .85 FTE (34 hours per week).

Requirements

  • Bachelors degree in social work, psychology or a related field.
  • Two years of social work/social services experience.
  • Positions within this classification may require licensure as a licensed professional counselor (LPC) or licensed social worker.

Nice To Haves

  • Bachelor’s degree in Social Work, Public Health, Nursing, or a related field required; Master’s degree.
  • Minimum of 2 years of experience in care navigation, case management, or public health.
  • Knowledge of HIV/AIDS and STI prevention, treatment, and care services.
  • Strong interpersonal and communication skills, with the ability to interact effectively with diverse populations.
  • Experience working with electronic medical records (EMR) and data reporting systems.
  • Ability to demonstrate cultural competence and sensitivity in patient interactions.
