About The Position

Provides emotional support and psychosocial counseling to patients. Assists in a multi-disciplinary approach to patient care. May identify discharge needs and arrange for comprehensive services for patients following discharge from the hospital. Performs related responsibilities as required.

Requirements

  • Bachelors degree in social work, psychology or a related field.
  • Two years of social work/social services experience.
  • Positions within this classification may require licensure as a licensed professional counselor (LPC) or licensed social worker.

Responsibilities

  • Hire, supplement, or sustain at least a minimal staff dedicated to RPC DPC services, demonstrating a consistent interdisciplinary team of excellent service
  • Obtain and/or maintain clinical skills competencies for evidence-based, best surveillance practices for high-risk infants, including HINE, AIMS, Bayley 4, gait analysis, autism screening, and parent counseling, and obtain CME/CEU credit to verify training
  • Collect & enter monthly data of HRIF to record provided neuromotor surveillance services
  • Utilize referral opportunities for enhanced care and services, including support from dedicated telehealth NICU follow-up advanced providers and DPC health coach
  • Host a minimum of one HRIF to Early Intervention Clinical Care Bridge Activity in your Region.
  • Clinical Bridge Activities will be administered by trained personnel to promote the development of child and developmental transition programs, activities, and clinical supports, and include:
  • Attendance at a state-wide RPC launch meeting to review the aims of the program and guidance for future steps
  • Attendance at early HRI annual conference, designed to align with community initiatives, with targeted clinical skills workshops and specialized knowledge
  • Completion of a brief REDCap survey on current allocated resources & processes for your RPC clinic
  • Hosting the Emory BBOP team at an early site visit to review survey responses and further discuss site challenges, needs, and support
  • Completing training on data entry for neuromotor surveillance using REDCap
  • Participation in monthly DPH RPC meetings and activities, with at least three representatives of RPC at each meeting
  • Completion of clinical efficiency and improvement initiatives, identified at monthly DPC meetings and supported through site meetings and 1:1 support.
  • These initiatives will include the tailoring of existing frameworks for the formalization of:
  • Supporting a master 's-level social work (SW) intern to assist in SW initiatives
  • Hosting the Emory BBOP team at a follow-up site visit to review progress and feedback, facilitators, and barriers to continued implementation
  • Training in the use of a health coach liaison to facilitate referrals to community resources to promote positive health.
