About The Position
The Integrated Care Management Program (iCMP) at MGH is looking for dynamic clinical social workers with excellent clinical, care coordination experience with strong organizational skills. The position is in a program of care coordination with a clinical social work focus, this role is embedded in a primary care at MGH within a multidisciplinary team. The role consists of assessing patients' needs and providing strong collaboration within the organization and community resources to provide individualized clinical interventions to patients and their families to provide optimal outcomes.
