About The Position

The Social Worker (HUD-VASH) position at the Department of Veterans Affairs in St. Louis is focused on providing intensive case management and psychosocial services to chronically homeless and/or disabled Veterans. This role is part of the HUD-VASH Program, which collaborates with HUD to help Veterans secure housing through Housing Choice Vouchers. The social worker will work within a multi-disciplinary team to enhance healthcare delivery for Veterans, ensuring their needs are met through comprehensive assessments and community resource coordination.

Requirements

  • Must be a U.S. Citizen.
  • Master's degree in social work from a CSWE accredited school.
  • Proficient in written and spoken English.
  • Must pass pre-employment physical examination.
  • Participation in seasonal influenza vaccination program is required.
  • Participation in the COVID-19 vaccination program is required.

Responsibilities

  • Conduct psychosocial assessments to determine needs of Veterans and family members.
  • Participate in discharge planning with other disciplines and ensure timely execution of discharge plans.
  • Provide consultation/education to Veterans and families on community resources, advance directives, and VA benefits.
  • Act as a liaison between VA and community resources, including marketing and public relations duties as needed.
  • Provide a full range of social work services including case work, individual, family, and group counseling, and case management.
  • Facilitate patient/family support groups and provide wellness/prevention education.
  • Assist patients and their significant others in coping with loss and grief experiences related to disability, terminal illness, and death.
  • Maintain data and statistical compilations to comply with Joint Commission, CARF, VA, and medical center policies.
  • Perform other related duties as assigned.
