About The Position

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. VA Careers - Social Work: https://youtube.com/embed/enRhz_ua_UU VA Careers - Licensed Clinical Social Worker: https://youtube.com/embed/U_xC25QsN0w Total Rewards of a Allied Health Professional This social worker functions as one of the Medical Center's Case Manager for Central Iowa Healthcare System for Homeless Veterans Program's HUD-VASH Program. The HUD-VASH program is a collaborative effort supported through HUD section 8 rental assistance vouchers and VA's provision of intensive case management services. They are responsible for program implementation and has ongoing contact with local Public Housing Agencies and providing case management-based services necessary to ensure that Veteran care is coordinated with the VA Medical Center, community health or mental health providers, and agencies that serve homeless population.

Responsibilities

  • Conduct psychosocial assessments to determine needs of veterans and family members.
  • Conduct intake substance use disorder assessments for patients to determine appropriate VA program needs.
  • Active participant in the treatment planning process with other disciplines.
  • Participate in transition/discharge planning with other disciplines and will be responsible for ensuring that transitions plans are executed in a manner that is timely and appropriate.
  • Provide individual, family and group counseling as appropriate. Individual and group services will be focused on substance use disorder treatment.
  • Provide consultation/education to veterans and families on community resources, advance directives and VA benefits.
  • Act as a consultant to other team/staff members.
  • Provide consultation/education to students of all disciplines on clinical practice and VA policy and procedure.
  • Provide case management to veterans and families, as needed, throughout the continuum of care.
  • Provide a full range of social services within commonly accepted standards of their profession, which includes case work, individual, family and group counseling, discharge planning, case management and other services as appropriate.
  • Provide wellness/prevention education and facilitate patient/family support groups as appropriate.
  • Coordinate treatment and communicate with the care lines to facilitate continuity of care for veterans.
