About The Position

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. VA Careers - Social Work: https://youtube.com/embed/enRhz_ua_UU The HUD-VASH Program is a partnership between the VA and HUD that is designed to help break the cycle of homelessness among Veterans. In this program, the VA Case Managers provide intensive case management and psychosocial services to chronically homeless and/or disabled Veterans and HUD provides a Housing Choice Voucher (HCV), also known as a Section 8 voucher, to supplement housing costs. The social worker works with VA Health Care System multi-disciplinary teams and community agencies to strive for developing more effective and efficient ways of delivering healthcare services to HUD-VASH Program Veterans. Duties include, but are not limited to:

Responsibilities

  • Conducts psychosocial assessments to determine needs of Veterans and family members.
  • Participates in discharge planning with other disciplines and will be responsible for ensuring that discharge plans are executed in a manner that is timely and appropriate.
  • Provides consultation/education to Veterans and families on community resources, advance directives and VA benefits.
  • Acts as liaison between VA and community resources, to include marketing and public relation duties as needed.
  • Provides a full range of social work services within commonly accepted standards of social work practice which includes case work, individual, family and group counseling, discharge planning, case management and other services as appropriate.
  • Provides wellness/prevention education and facilitate Patient/family support groups as appropriate.
  • Assists patients and their significant others with coping dealing with the loss and grief experiences in disability, terminal illness and death.
  • Maintains data and statistical compilations to comply with Joint Commission, CARF, VA and medical center and service policy and procedures.
  • Performs other related duties as assigned.
