Requirements

  • Master's degree in Social Work from an accredited institution.
  • Licensure or certification to independently practice social work at the master's degree level.
  • Proficiency in written and spoken English.
  • Experience in discharge planning and case management in a healthcare setting.
  • Knowledge of medical terminology and mental health diagnoses.

  • Doctoral degree in Social Work.
  • Experience working with Veterans or in community settings.
  • Familiarity with VA performance measures and accreditation standards.

Responsibilities

  • Provide independent case management for a caseload of Veterans in the HUD-VASH program.
  • Deliver a full range of clinical services including case work, individual, family, and group counseling, and discharge planning.
  • Conduct outreach to identify and screen Veterans for mental health and housing needs.
  • Implement evidence-based psychotherapeutic interventions and measure outcomes.
  • Perform psychological assessments and diagnose mental disorders in complex cases.
  • Develop treatment plans in collaboration with Veterans and interdisciplinary teams.
  • Coordinate community-based services and referrals to additional resources.
  • Identify high-risk patients and provide crisis intervention and case management services.
  • Participate in staff meetings, trainings, and compliance with local policies and procedures.
  • Maintain data and statistical compilations for accreditation and performance measures.
