About The Position

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. The incumbent provides the appropriate level of case management and supportive services, primarily in the community or in the home, to Veterans based on their needs and wants. Making regular home visits, based on the acuity level of the Veteran, to assess Veterans' housing stability, social and community integration, and recovery process. Additionally, home visits help ensure the residences are safe environments and in compliance with HQS. You must possess a valid driver license. The incumbent provides training for other members of the multidisciplinary treatment team in areas related to SUD assessment, anger management, life skills, and SUD with co-occurring disorders. The incumbent independently conducts intake and needs assessment of homeless Veterans and their significant others, as well as Veterans already housed with HUD/VASH. The incumbent identifies symptoms leading to diagnoses of SUD. The incumbent provides all necessary assessment functions, e.g., interviewing, histories, and SUD assessments to aid in the development of treatment plans. The incumbent develops comprehensive treatment plans with the Veteran that address the factors that contributed to homelessness, including SUD and other co-occurring factors within a recovery model framework. Incumbent works with the Veteran to realize the Veteran's treatment goals and reassesses and revises goals as indicated by the Veteran's engagement in treatment. The incumbent organizes and conducts individual and group SUD treatment, to include early interventions and supportive follow-up to ensure stability in home visits and facility-based contacts. Incumbent provides SUD support to the HUD-VASH team as a functional member of this team, to include consultation, interventions, or accompanying other case managers on home visits, when indicated by suspicion of HUD-VASH participant SUD use. The incumbent serves as a member of a multidisciplinary homeless program treatment team. Incumbent maintains a caseload as determined by HUD/VASH Coordinator, Manager, & Service Chiefs. The incumbent will develop partnerships with community agencies, especially those serving homeless and SUD Veterans, with regular contact and communication. The incumbent will provide liaison services with other programs referring homeless Veterans to HUD-VASH to ensure continuity of care and promote successful transition of Veterans from one program to another. The incumbent will provide consultative services within HUD-VASH, as needed. Incumbent will participate in policy making, goal setting, and treatment service planning discussions with other members of HUD-VASH and Homeless Programs.
