About The Position
The Social Worker HUD-VASH Case Manager position is a vital role within the Veterans Health Administration, specifically designed to provide comprehensive support and case management services to veterans experiencing homelessness. This position is responsible for program oversight, monitoring, and administration, serving as the primary contact with local Public Housing Authorities. The case manager will ensure that veteran care is effectively coordinated with the VA medical center, community health or mental health providers, and various agencies that serve homeless populations. In this role, the incumbent will develop psychosocial treatment plans that actively involve veterans and their families, working closely with interdisciplinary team members based on thorough psychosocial assessments. The case manager will utilize their professional skills, clinical training, and experience to interpret data, identify viable treatment options, and recognize potential high-risk factors and needs for services. Regular visits to veterans in their residences will be conducted to assess their environments and provide ongoing clinical services, fostering a therapeutic relationship that values the veteran's input in their care. The case manager will assist veterans in maintaining appointments at VA medical centers and other community agencies for various services, including medical, psychiatric, substance abuse, and vocational support. They will implement the Housing First model within the HUD-VASH program, focusing on the direct placement of targeted homeless veterans into permanent housing. The position also involves documenting all interactions and treatment plans in medical records, managing quality control, and providing educational services to veterans and their families. This role is crucial in promoting healthful independent living for veterans and ensuring they receive the necessary support and resources.
