: Full time Social Worker - Hospice Columbia and Greene Counties
About The Position
The Community Hospice has a full time opportunity for a social worker to join our hospice care team caring for patients in primarily the Columbia county area as part of our Interdisciplinary Team in homes and Nursing homes. As a member of the hospice interdisciplinary team, the primary focus is to respond to the emotional and psychological needs of patients, families and staff. Position is Full time Monday- Friday 8a-4:30p. Requirements
- NYS SW Licensure
- Valid NYS Drivers license
- Minimum 2-3 years experience ideally caring for seniors or those with advance illness
Responsibilities
- Diagnose and treat psychological, behavioral and emotional disorders as they relate to end of life care
- Provide specialized skills and mental health therapy for clients
- Identify barriers which impact optimal patient functioning
- Ensure patients receive appropriate resources in most appropriate setting
