About The Position

The Social Worker uses experience knowledge, judgment and skills to provide social services to patients and families in both complicated and uncomplicated situations. The Social Worker maintains patient confidentiality and promotes patient rights as supported by agency, state and federal directives. Identifies cultural diversity and lifestyle choices and non-judgmentally accommodates for such differences. Additionally they provide consultation to other hospice staff and volunteers while consistently demonstrating the Hospice Savannah values and compliance with organization’s service excellence standards.

Requirements

Graduate of an accredited school of social work with Master’s degree in Social Work required, licensed Social Worker required.
Must have a minimum of 2 years social services experience, family work and healthcare experience preferred.
Shall adhere to and promote the ethics and values of the Social Work profession using the NASW Code of Ethic as a guide to ethical decision making in social services practice.
Valid driver’s license, automobile insurance and reliable transportation required.
Excellent communication skills (written and verbal) and the ability to successfully interact with a diverse patient population, families, physicians, Hospice Savannah staff, and the community at-large along with excellent customer service and documentation skills required.
Computer proficiency in Microsoft Office, including Word and Outlook, and database knowledge required.
