About The Position

The Social Worker for Hospice at Trinity of El Paso Home Health & Hospice is responsible for providing medical social work services in accordance with established standards of care. This role involves collaboration with primary care physicians and healthcare teams to support clients and their families in navigating personal and environmental challenges that affect their medical care. The position is per diem, allowing for flexible scheduling based on client needs.

Requirements

• Master of Social Work (MSW) degree required.
• Experience in medical social work services preferred.
• Strong communication skills for effective interaction with clients, families, and healthcare teams.
• Ability to perform socio-psychological evaluations and develop care plans.

Nice To Haves

• Experience in hospice or palliative care settings.
• Familiarity with community resources and support services for clients.

Responsibilities

• Receive intake referral information and prepare necessary paperwork/tools.
• Obtain pertinent medical history from clients, families, or significant others.
• Perform socio-psychological evaluations of support systems and document emergency contacts.
• Participate in the development of care plans and discharge planning.
• Assist clients and families with personal and environmental difficulties that interfere with medical care.
• Perform skilled visits and document accordingly.
• Provide supportive casework to restore clients to their optimum level of social adjustment.
• Assist clients in utilizing family and community resources.
• Effectively communicate with clients, families, and other healthcare disciplines.
• Submit accurate and complete paperwork weekly.
• Coordinate community services for safe home care or hospice needs.
• Participate in QA Committee processes as requested.
• Act as a consultant to other agency staff.
• Notify the agency of emergencies or occurrences affecting client caseload.
