About The Position

The Social Worker plays a crucial role in assisting patients and their families by addressing social, economic, psychological, and spiritual needs. This position involves utilizing community resources to minimize stressors and problems that arise from health-related issues. The Social Worker conducts assessments, provides counseling, and coordinates with various community services to support patients and their families throughout their healthcare journey.

Requirements

  • Master's degree in Social Work (MSW).
  • At least 1 year of experience in a social work role within a healthcare setting.
  • BLS or ACLS certification.

Nice To Haves

  • Experience in hospice care or palliative care settings.
  • Familiarity with community resources and support services for patients and families.

Responsibilities

  • Interview patients, family members, and hospital personnel to identify and address needs.
  • Assess the psychosocial status of patients related to their illness and environment, communicating findings to case managers and clinical managers.
  • Provide counseling and support on issues related to death, dying, grief, and loss.
  • Educate patients and families on resources, medical conditions, the normal grief process, organ donation, and financial management.
  • Make referrals and coordinate with community resources such as clergy, family services, and government programs.
  • Visit patients in their homes to assess their psychosocial needs, including social history and coping skills.
  • Participate in interdisciplinary conferences on behalf of patients and contribute to individual patient care plans.
  • Provide support to patients and families through individual or group conferences.
  • Prepare and maintain current case records for each patient, including evaluations and treatment plans.
  • Attend educational programs and professional meetings related to social work services.
  • Assist team members in understanding significant social and emotional factors related to health problems.
  • Stay informed of changing Medi-Cal and Medicare regulations and follow medical recommendations for optimal patient adjustment.
  • Participate in hospital performance improvement activities.
