About The Position

The Social Worker will work in collaboration with a care team, providing patient and family advocacy and psychosocial interventions for patients receiving services thru Hospice Care, Home Health, and Private Duty programs. Must strive to promote patient and family wellness and comfort through improved care outcomes, completion of advance care planning, referrals to community resources and end of life care issues and/or access to post-acute physician services. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.

Requirements

  • Social Work assessment and counseling skills.
  • Knowledge of state and federal programs that provide medical care and financial support.
  • Completion of an accredited BSW program.
  • Current licensure as a BSW in the State of Kansas.
  • BLS – BLS is required within three months of hire.
  • Valid Kansas State Driver’s License.
  • Auto Insurance equal to or greater than the Kansas insurance requirements.

Nice To Haves

  • Completion of an accredited MSW program.
  • 1-3 years’ experience in medical facility, hospice and/or home care.

Responsibilities

  • Systematically gathers pertinent psychosocial data for patients whereby patient and family psychosocial needs can be identified and appropriate interventions performed.
  • Conducts home visits to perform a psychosocial assessment for the patient and family, incorporating social, cultural/ethnic, emotional and spiritual aspects of care.
  • Develops psychosocial plan of intervention utilizing all available sources of information.
  • Develops a focused plan of intervention based upon the psychosocial assessment in collaboration with the interdisciplinary team when appropriate and based upon the patient identified needs.
  • Incorporates social, emotional and spiritual aspects of care into the patient’s plan.
  • Performs assessment/data collection in an ongoing and systematic manner, focusing on psychosocial, self-care, education, and environmental factors.
  • Uses pertinent performance metrics to drive improvements in practice that will enhance patient or consumer safety, satisfaction, efficient and effective care and that will identify and promote best practices and equitable care on a multidisciplinary basis.
  • Collaborates with physician, patient/family and other assigned disciplines regarding care needs, formation of and revisions to plan of care.
  • Treats patients and their families with respect and dignity.
  • Advocates for needs, decisions, and rights of patients and families.
  • Provides liaison function between outreach programs and hospice, home health, long term care facilities and other community services as needed.
  • Facilitates advance care planning, addressing end of life issues, healthcare proxies, living wills, advance directives, and power of attorney.
  • Stays abreast of changing care management and Social Work clinical trends, regulatory matters and payer requirements.
  • Establishes and maintains positive working relationships with patients, family members, co-workers, physicians and referral sources.
  • Completes timely and accurate documentation as per agency policy.
  • Abides by the Health Insurance Portability and Accountability privacy and security regulations regarding all aspects of Protected Health Information (PHI).
