About The Position

The Social Worker (Homeless Outreach) position at the Coesville VA Medical Center focuses on providing recovery-oriented services to homeless veterans and their families. The role involves conducting psychosocial assessments, developing treatment plans, and coordinating care with various disciplines to ensure veterans receive appropriate support and resources. The incumbent will also serve as a consultant and advocate for veterans, facilitating access to VA healthcare services and community resources.

Requirements

  • U.S. Citizenship is required.
  • Master's degree in social work from an accredited school of social work.
  • Licensure or certification to independently practice social work at the master's degree level.
  • Proficiency in spoken and written English.

Responsibilities

  • Identify homeless veterans in the medical center and community, ensuring eligible veterans are enrolled for VA healthcare services.
  • Conduct psychosocial assessments to determine the needs of veterans and their family members.
  • Participate in treatment planning with other disciplines.
  • Engage in discharge planning and ensure timely execution of discharge plans.
  • Provide individual, family, and group counseling as appropriate.
  • Educate veterans and families on community resources, advance directives, and VA benefits.
  • Act as a consultant/liaison to other team members and programs to ensure appropriate care for homeless veterans.
  • Provide a full range of social work services including case work, counseling, and case management.
  • Facilitate wellness/prevention education and support groups for veterans and families.
