Homecare/ Hospice: Provides supportive services to help meet the patient/family social, emotional, economic, and environment needs in relation to the impact of a chronic, acute, or terminal illness though education, support/counseling, and referrals/coordination of community resources. Completes timely visits to meet patient/family needs and ensure program compliance. Collaborates with the interdisciplinary team to provide whole person care addressing bio-psycho-social needs of patients and family. Provide patients/families with the psychosocial support needed to cope with chronic, acute or terminal illness through education, support/counseling and referrals for community resources. In addition to conducting community visits, Social Workers may complete consultation for patient’s inpatient at Lakeview Hospital to provide education about homecare, palliative care, and hospice.
