About The Position

The Medical Social Worker at CenterWell Home Health plays a crucial role in providing interdisciplinary care to home health patients. This position involves evaluating and developing personalized care plans that address the emotional and social needs of patients. The Medical Social Worker also supervises Social Worker Assistants and collaborates with healthcare teams to ensure comprehensive patient care, while adhering to company policies and healthcare regulations.

Requirements

• Master's or doctoral degree from an accredited school of social work.
• Social Worker licensure in the state of practice, if required by state law.
• Valid driver's license, auto insurance, and reliable transportation.
• Current CPR certification.
• Minimum of one year of experience as a social worker in a healthcare setting, preferably in home health or hospice.
• Knowledge of discharge planning needs and community resources.

• Experience in hospice social work.
• Strong communication and interpersonal skills.

Responsibilities

• Evaluate and develop personalized care plans for patients based on their emotional and social needs.
• Supervise and provide direction to Social Worker Assistants involved in patient care.
• Assess patients' social and emotional states in relation to their illnesses or injuries.
• Identify and coordinate community resources to assist patients in their recovery.
• Instruct patients and families on coping with social and emotional responses related to health issues.
• Participate in the development and periodic re-evaluation of the physician's Plan of Care.
• Observe, record, and report changes in patients' conditions to the Clinical Manager.
• Participate in discharge planning processes and care coordination activities.
• Maintain and submit required documentation, including visit reports and progress notes.
