About The Position

Social Services is now accepting applications to be considered for a Full-Time Social Worker in the Home and Community-Based Services Unit. Requirements

• Bachelor’s degree from an accredited four-year college or university with a major in Social Work, Psychology, Sociology or closely related field.
• 1-3 years prior work-related experience
• Valid driver’s license
• Must be a current Clay County Social Worker
• Successfully complete a background investigation

Responsibilities

• Provide case management and support coordination for adults with disabilities and home- and community-based service programs.
• Assess individual and family needs, develop and implement service plans, and coordinate services to promote independence and community living.
• Collaborate with interdisciplinary teams, including health care providers, and community resources, to ensure integrated and person-centered care.
• Monitor and evaluate ongoing service effectiveness and client progress, ensuring compliance with state and federal program requirements.
• Maintain accurate and timely documentation in accordance with agency and state guidelines.
• Participate in multidisciplinary team meetings and county/state trainings as required.
• Other duties as assigned.
